Enhancing antigen sensitivity of bcma car t cells for durable clinical responses inmultiple myeloma - project summary the prognosis for patients with multiple myeloma is dismal, highlighting the urgent need for more effective therapies. Bcma car t cell therapy has shown clinical promise in multiple myeloma; however, IT has failed to sustain long-term responses with a 5-YEAR progression free survival of only 21%. One mechanism of relapse and resistance to bcma car t cell therapy is low and heterogenous expression of the target bcma antigen on cancer cells within and across patients. As cars require much higher antigen expression than natural t cell receptors for activation, cancer cells with low bcma expression can escape car t cell surveillance. The overall objective of this Phase I STTR project is to nominate a lead car design to translate into a next- generation bcma car t cell therapy for multiple myeloma. The lead car design will be identified using high- throughput in vitro and in vivo screens with cancer cells expressing low levels of bcma. The central hypothesis is that car design optimization can enhance antigen sensitivity. To achieve this overall objective, the research proposal is structured into three specific aims focused on (1) screening >20,000 car designs for improved antigen sensitivity in vitro, (2) stringently evaluating the top 2,500 car designs via in vivo ...
